Being Wise Keeping Well
A preventative scheme for older people who may need practical help, information or support to reduce isolation, improve health or find out what is available in the local community 01257 479023.
Carers Access to Health
The Carers Access to Health Project supports people who have a caring role, to attend health related appointments. The carer or the cared for must be over 65 years. The service can proved a sitting service whilst the carer attends an appointment and can assist the carer and/or cared for to attend appointments. This service is provided free of charge. Charity Shop
Our Charity Shop located at 35 Chapel Street, Chorley PR7 1BU (tel 01257220153), which offers a wide range of quality, nearly new clothing, accessories and bric-a-brac. Using our shops gives you the opportunity to find a bargain whilst helping generate income and recycle your old unwanted items by donating them to the shop.
Chorley Older Peoples Forum
Established and supported by Age Concern Lancashire, the Chorley OIder Peoples Forum is an important means of letting agencies such as the District and County Councils and Social Services know what is important for older people. If you would like to attend a meeting or become involved contact the forum co-ordinator on 01257 479018.
Community Links - Assisted Shopping Scheme
Our volunteers offer short term support with a view to helping older people re-engage with the local community, and offer practical support where necessary. Our supported shopping service is available to older people who cannot shop for themselves due to injury or illness, or for older people who may find it difficult to shop alone. Tel 01257 479005.
Community Support, Leisure and Social Activities
Our team of Outreach Officers provide support in many rural districts of Chorley as well as in Coppull and Clayton Brook. They work to identify what is needed in local neighbourhoods and look at ways of setting up services and activities to meet those needs. Opportunities are available to provide a range of activities such as luncheon clubs, games and craft sessions, computer classes and home tuition sessions. Flexible Outreach Service
This service enables individuals in the Chorley area with early stage memory problems to maintain or re-develop daily living skills, have opportunities to be involved in educational, leisure or creative activities and be supported in maintaining or developing contacts in their local community. Trained staff provide an individualised home visiting service. For more information contact the Co-ordinator on 01257 479024.
Foot Care Service
This is a partnership project between the NHS Podiatry Service and Age Concern Lancashire. The scheme provides a nail clipping service carried out in the clients own home, advice on how to improve and maintain general foot hygiene and a reassuring check to make sure there are no potential foot care problems. Health Trainer
This new project promotes and supports older people to have a healthier lifestyle. Our Health Trainers offer practical support and encouragement to older people to change their lifestyles and give them information about activities taking place in the local communities. Help Direct
Help Direct is a new service being developed for all adults in Lancashire. Help Direct will aim to be your first port of call to get the right practical support, guidance and/or information you need to get the most out of life. You can call the low call phone number or drop in to speak to someone face to face. Your local Help Direct Team will help you find the right way forward – one that works for you. Home Help Service
The new Home Help Service is available to all people aged 50 and over living in the Chorley area. Home Helps are able to carry out a variety of tasks including general cleaning, laundry and run local errands such as posting letters. Hospital Aftercare Scheme
Our team of staff offer practical support in the home following discharge from a hospital ward or Accident and Emergency Department at Chorley General Hospital. People must be aged 60 or above and finding it difficult to resume their activities of daily living. Support can be planned for up to 8 weeks.

Information and Advice
Our staff and volunteers are trained to provide information on a range of topics and to give up to date fact sheets or signpost to a more relevant agency. Home visits can be arranged.
